Book Abstract

1.定义：漏斗胸（funnelchest）是指胸骨连同肋骨向内向后凹陷，呈舟状或漏斗状；胸骨体剑突交界处凹陷最深。
2.病因：漏斗胸形成与肋骨生长不协调或膈肌中心腱过短向后牵拉胸骨和剑突有关。此畸形有家族倾向或伴有先天性心脏病。
3.临床表现：胸廓畸形及相关的压迫症状。压迫较轻微时可无明显症状。患儿常体形瘦弱，少动，易患上呼吸道感染，活动能力受限，活动后出现心慌气短及呼吸困难。
4．临床诊断：查体可见胸廓畸形，常合并轻度驼背和腹部凸出等特殊体型，可有呼吸次数快，吸气性喘鸣，心率快等体征，大龄患儿发育可受限。x线侧位胸片可见下段胸骨向后凹陷。胸部CT可确切清晰地显示胸廓畸形。
5.治疗：除畸形较轻微者以外应予以手术治疗，早期手术效果好，3～4岁即可手术矫治。
5.手术方法：胸肋抬举术或者胸骨翻转术。
第二节非特异性肋软骨炎
1.定义：非特异性肋软骨炎，即Tietze病，是一种非化脓性肋软骨肿大。女性发病略多，多为单侧，2～4肋软骨。
2.病因：病因不明，可能与劳损、病毒感染等有关。病理切片多无异常。
3.临床表现：局部肋软骨轻度肿大隆起，局部压痛，咳嗽、上肢活动时加重。病程长短不一，可时轻时重。预后良好。
4.临床诊断：根据症状、病史和查体除外胸内病变、肋骨结核或骨髓炎等，可诊断。x线片因肋骨不显影，对诊断此病无帮助。5．治疗：对症治疗。病程长、疗效不佳且症状重者或不能排除肿瘤者，可切除病变部位肋软骨。
