Book Abstract

【定义】
秋燥是在秋季感受时令燥气之邪，以肺系为病变中心，津气干燥为基本特征的急性外感疾病。病变性质属热者称为温燥，属寒者称为凉燥。但均具有口、鼻、唇、咽等清窍干燥的表现，在病程中燥邪易化火伤阴，损伤肺络。但一般传变较少，整个病程不长。
【历史沿革】
秋燥病名，最早见于明代喻嘉言《医门法律·秋燥论》。但有关燥气致病则旱在《内经》就有论述。《素问·气交变大论篇》有“岁金太过，燥气流行”的记述，已经认识到燥气能致病，并和时令气候变化密切相关。《素问·六元正纪大论篇》说：“凡此阳明司天之政……三之气，天政布，凉乃行，燥热交合，燥极而泽，民病寒热。”《素问·阴阳应象大论篇》也有“燥胜则干”的记载。这些都初步指出了燥气致病的临床特点，为后世研究和论述秋燥病提供了理论依据。
汉、唐时代，困于《内经》病机十九条中未提及燥气为病，故医家对秋令燥气为病的阐述不多。直至金元时代，刘完素在《素问玄机原病式》中，才针对《素问·至真要大论篇》病机十九条中缺乏燥气致病条文的情况，补充了“诸涩枯涸，干劲皴揭，皆属于燥”一条，虽然不是专指外感燥气而言，但却从一般意义上阐明了燥气致病的临床特点。刘氏并指出治疗“宜从退风散热，活血养液，润燥通气之凉药润之”，对秋燥病的理论建立作出了重要贡献。
对秋燥进行深入研究，并作出系统论述的，始于明代喻嘉言之《秋燥论》。在该书中，喻氏根据临床实际的情况，把《内经》“秋伤于湿”之说改为“秋伤于燥”。并指出燥气性质同于火热。他说：“燥金虽为秋令，虽属阴经，然异于寒湿，同于火热。”又说：“凡秋月燥病，误认为湿治者，操刃之事也。”治疗上则针对燥热伤肺的特点，创制了著名的清燥救肺汤，从而奠定了治疗温燥病的基础。此后，清代沈明宗有“燥属次寒，感其气者，遵《内经》‘燥淫所胜，平以苦温，佐以辛甘’之法，主用香苏散加味，此治秋伤凉燥之方法也”（何廉臣著，《重印全国名医验案类编·燥淫病案》），从而开创了治疗凉燥的先河。
清代，随着温病学的发展，对秋燥病的认识也日益深刻和全面，一些著名的温病学家如吴鞠通、俞根初等，在继承前人理论和经验的基础上，对秋令煤气的性质及其引起的秋燥病的病机、证型，提出了较为全面的看法。俞根初在《重订通俗伤寒论·秋燥伤寒》一节中指出：“秋燥初凉，西风肃杀，感之者多病风燥，此属燥凉，较严冬风寒为轻。若久晴无雨，秋阳以曝，感之者多病温燥，此属燥热，较暮春风温为重。”由此可知，秋令燥气既可从热而化为燥热之邪，亦可从寒而化为凉燥之邪。燥而偏热的是为温燥，燥而偏寒的是为凉燥。在辨证论治上，俞氏又根据温燥和凉燥的不同类别，及其发展过程中对各个脏腑功能的影响，提出了一整套的治疗方药，使秋燥病在证、因、脉、治各个方面，均有了较为系统的认识。
【范围】
根据本病的发病季节和临床特点，秋燥病包括了西医学中发生于秋季的上呼吸道感染、流行性感冒、急性气管～支气管炎、肺炎等感染性疾病，出现秋燥见症时，可按本篇内容辨证论治。
【病因病机】
秋燥病因是感受时令燥邪，其病位与肺金内应，亦可内传入里如阳明经府，甚或内陷心营。病理因素是燥邪，病机特点是燥胜则干，易伤津液，亦易化火，故其病机性质初为表实，病久则为里虚。
1．肺卫先病秋燥病的致病因素是外感时令燥邪。燥为秋令主气，具有干燥的特性，但其致病，在性质上则有燥从热化的温燥和燥从寒化的凉燥之分。这主要与秋令气候的冷暖变化有关。但凉燥之邪亦可因患者之体质偏热而从热化，其性质则与温燥相同。肺为燥金，燥气内应于肺。燥邪侵袭人体多从口鼻而入，先犯上焦肺经。肺主气属卫，肺受燥侵，外则卫气失和，内则肺气失宣。其病机凉燥与风寒感冒相近，温燥与风温类同。正如叶天士所说“温自上受，燥自上伤，理亦相等，均是肺气受病”。所不同者，秋燥病尚有“燥胜则干”的病机变化，这是本病的特点。所以秋燥初起，除其有邪袭肺卫的一般外感见证外，必然伴有口、鼻、唇、咽干燥及干咳等肺系津液干燥征象。
2．伤阴化火“水流湿，火就燥”，燥邪易化火。燥邪本易消耗津液，一旦燥邪化火，则更易劫灼阴液，甚或损伤血络。故燥邪侵肺后，如不能及时清解，则每易导致肺燥化火，耗劫肺阴，灼伤肺络，而产生肺燥阴伤和络损出血之变。后期即使燥热已清，但肺阴损伤多难骤复，故常有肺燥作咳之证遗留；肺与大肠相表里，亦有因肺燥而致大便燥结难行的。
3．内传入里秋燥病以上焦肺经病变为主，一般治疗及时，燥邪即可由此而解。但亦有因邪势严重，冶不及时或治不得法，致肺燥之邪未得外解而内传入里。其顺传中焦者，病机变化亦不外阳明经、腑二证，这与风温邪传中焦基本相同。只是秋燥的阳明实热证候每伴有津液受损的表现。燥热化火内陷心营者，其病机亦与风温内陷心营相同，可见热灼营阴，甚或耗血动血及闭窍神昏之变，不过一般较为少见。
