随着全膝关节胃换术（total knee arthroplasty，TKA）技术的日趋成熟，TKA术后效果越来越满意，使得这一技术的手术适应证有进一步扩大趋势，并逐渐应用于更多年龄较轻的终末期膝关节疾病患者。但术后膝关节屈伸活动度是患者TKA术后对手术满意度的重要因素之一，为满足日常生活和体育活动需要，常常要求膝关节屈曲达到120度以上，对于年龄较轻的患者，尤其是东方人，工作和生活的需要使得他们对膝关节屈曲活动的要求更高（如图1-1a、b、e）。而传统的TKA术后膝关节屈曲很少超过120度，因此，TKA术后膝关节获得理想的屈伸活动范围已成为医生患者极为关注的问题。
现代TKA术后屈曲活动受限（大于85度）的发生率可高达8%到12%，其影响因素主要包括患者因素（术前活动度、肥胖、伴随疾病），假体的设计与选择，手术操作技术（屈伸间隙平衡、力线、假体的大小与位置），术后并发症（术后感染、异位骨化、深静脉血栓等），术后疼痛以及其他如合并腰椎病变、交感神经反射性萎缩等。
