第一节 应激的概念
一、应激概念的形成
应激（stress）作为一个术语最早出现于物理学中，指作用于某物之上的足够使其弯曲或折断的拉力，即作用力与抵抗力的综合关系的“应力”，我国学者译为“应激”。20世纪30年代，加拿大病理生理学家塞里（HansSelve，1907——1982）首次将“stress”引入医学领域，提出了应激概念。1936年塞里在Nature杂志上发表了第一篇有关应激学说的文章——《由广泛的致病因子引起的一种综合征》。他描述了由不同种类的应激源引发的病理三联症（肾上腺肥大、胃溃疡形成和胸腺淋巴管萎缩），塞里将机体的这种状态称为“应激”，定义为机体在任何需要下的非特异性反应（从总体反应中除去特异的组成部分），强调接触任何应激源，均会导致同样的病理三联症——应激综合征。机体在此状态下的种种征候表现叫做适应综合（adaptationsyndrome）或应激综合征，由此而引起的疾病叫适应性疾病。根据塞里的理论，这些作用于机体的刺激包括：感染、中毒、创伤、高温、受冷、电离辐射、强烈的精神刺激以及剧烈的肌肉运动等。塞里的工作不仅促进了垂体、肾上腺的研究，也推动了整个内分泌理论的发展。应激学说促进了临床内分泌学的发展，也为临床广泛应用激素作为治疗手段奠定了理论基础。
20世纪70年代，Mason和Frankenhauser等观察到，应激反应不仅取决于应激源的质量，而且还与机体对应激源的认知和评价密切相关。因此，有关应激的概念进一步扩展，认为应激反应还是有特异性的，对应激的发生提出了是由躯体和心理双重因素诱导，外源性应激源影响与机体内在素质双重因素决定应激强度的理念，也就是说，应激不仅取决于环境的刺激，还取决于个体的心理特征；应激不仅引起神经内分泌变化，还引起行为变化，这种变化是防御性的，但也可以导致病理性行为异常。因此对应激的研究，应从对环境变化的感知、由此引起的情绪变化、神经内分泌反应及行为变化等方面进行。

儿童青少年占总人口的三分之一，任何社会变革和生活环境的改变都会给儿童的生存与发展带来重要而深远的影响。显而易见，改革开放以来我国社会经济高速发展、城市化速度加快、竞争及工业污染加剧等问题的负面因素已影响到人群中最脆弱的群体——儿童的发展。其次，社会发展也带来一些值得关注的负面影响，如当下应试教育催生的对学生的高压和高期望，家长和教师的简单粗暴教育方式，校园暴力，儿童虐待与忽视，一些难以预测的天灾人祸等均构成儿童生活中的重大生活事件，势必给儿童青少年造成不同程度的创伤性体验，甚至是更为严重的身心摧残，这使得儿童创伤后应激障碍（posttraumatic stress disorder，PTSD）成为当今临床心理学研究的一个热点问题。我们还需关注的一些问题是，医学科学的长足进步和医疗水平的提高，使高危出生史儿童存活率大为提高，出生缺陷和发育缺陷相关疾病的发生率逐年上升，而他们的后期发展显然容易出现各种发育行为问题；研究发现这些儿童具有较高的生物易感性和脆弱性，他们比起正常发育儿童更加不耐受各种应激性事件的打击。
