Author Description

海因茨·科胡特（Heinz Kohut，1913-1981），自体心理学创始人，前美国精神分析协会的会长、前国际精神分析协会的副会长、前西格蒙德·弗洛伊德档案馆副馆长以及奥地利科学院成员。曾获得奥地利政府颁发的荣誉十字勋章。他坚持认为心理缺陷而非冲突是许多人患病的原因，是从20世纪70年代以来对传统美国自我心理学的主要挑战者。

许豪冲，台大医学系毕业、精神科专科医师、台北市立仁爱医院精神科主任。

Book Abstract

在各种情况下，有一个问题一直考验着分析师，那就是每当要结束一个分析的时候，分析的任务是否已经完成，或是这样的结案是否过早。除此之外，还有一些特定的问题环绕着自恋型人格分析的结案。下面的事实增加了结案的复杂性：分析师所持的观点不但关系到理论与实务的很多领域，也会影响分析师自己的判断，对于应该如何定义理想地结束分析以及期待的现实结果应该离理想多近的判断。所以，结案是个很大的议题。在本书的研究里，我将会忽略这个问题的很多面向，而只局限于尝试澄清若干理论问题。我之所以执行这个任务，是因为我相信我们传统理论的立足点的改变，将使我们能够认知某些结案的真实性（genuineness），使我们能够认知更进一步的分析是不必要的，以及因为我相信病人并非隐匿病情，伴作健康以逃避分析（flight into health）——然而基于传统理论所做的病人人格的评估，可能会引导我们有相反的看法。

然而，当我们转而面对自恋型人格障碍，我们要处理的不再是基本上完整的结构，不是处理它们之间的冲突的未满意解决的病理结果，而是因为人格的中心结构——自体的结构——有缺陷而产生不良的心理功能形式。所以，就自恋型人格障碍而言，我们如果要描述其精神分析的过程与目标，以及标志其真正结案（在这种情形下，我们可以说分析的任务已经被完成）的情况，必须基于根本的心理缺陷的本质、位置，及它们的治愈的定义。

Introduction

在几个方向上，这本书超越了我之前关于自恋的作品。在以前的书中，我主要以古典驱力理论的语言来提出自体心理学的发现。本书所引入的关键理论概念就是“自体-客体”（self-object）；而在治疗领域中与自体-客体概念相关联之最重要的实证发现，就是我描述的自体-客体移情的现象。最后，当联结理论与临床观察，以及重建发展与治疗的理论，先前的作品引入转变内化作用（transmuting internalization）的概念，与自体领域的结构形成相关联的理论。
