小续命汤
【来源】《备急千金要方·卷八》
【组成】麻黄一两防己一两人参一两黄芩一两桂心一两甘草一两芍药一两川芎一两杏仁一两附子一枚防风一两半生姜五两
【用法】以水一斗二升，先煮麻黄三沸，去沫，纳诸药，煮取三升，分三服，良；不愈，更合三至四剂，必佳。取汗随人风轻重虚实也。
【功用】扶正祛风。
【主治】正气内虚，风邪外袭。诸风服之皆验，不令人虚。中风卒起，不省人事，神气溃乱，半身不遂，筋急拘挛，口眼喁斜，语言謇涩，牙关紧闭，厥冷；或顽痹不仁，风湿腰痛。卒中风欲死，身体缓急，口目不正，舌强不能语，奄奄忽忽，神情闷乱。中风不省人事，涎鸣，反张，失音，厥冷。八风五痹，痿厥。产后中风。风湿腰痛；痰火并多，六经中风，及刚柔二痉。
【方解】本方为麻黄汤合桂枝汤加味组成，麻黄汤疏达肌表，桂枝汤调和营卫，藏刚柔相济之妙，寓表里互调之功；麻黄、桂枝散瘀通脉，防风、防己祛风胜湿；人参益气补中，白芍养血和营；黄芩清热，附子散寒，杏仁宣肺祛痰，生姜温胃止呕，甘草和中调药，共治风寒入络，血凝脉涩之证。
【临床应用】
1．随症加减无汗恶寒，倍麻黄、杏仁。有汗恶风，倍桂枝、芍药。无汗身热不恶寒，去附子倍甘草加石膏、知母。有汗身热不恶风，倍桂枝、黄芩加葛根。无汗身凉脉沉细，恍惚者，加茯神、远志。如骨节烦疼，本有热者，去附子，倍芍药。有汗无热畏寒脉沉，倍桂枝、附子、甘草。肢节挛痛，麻木．不仁，脉缓，加羌活、连翘。
2．历代名家的应用经验
（1）《千金方衍义》：小续命汤虽本古方，而麻黄、桂枝两方皆在其中。以其本虚，必加人参驾驭麻、桂，发越在表之邪，又需附子直人少阴，搜逐在里之邪，不使外内交攻，正气立断，续命之名，信乎不虚。其余川芎、黄芩、防风、防己，不过为麻黄之使，以祛标热耳。方治卒中风欲死，病死于暴，故用麻黄必兼杏仁开发肺气之逆满，殊不可缺。
（2）《医方考》：麻黄、杏仁，麻黄汤也，仲景以之治太阳证之伤寒；桂枝、芍药，桂枝汤也，仲景以之治太阳证之中风。中风而有头疼、身热、脊强者，皆在所必用也。人参、甘草，四君子之二也，《局方》用之以补气；芍药、川芎，四物汤之二也，《局方》用之以养血。中风而有气虚、血虚者，皆在所必用也。风淫未疾，故佐以防风；湿淫腹疾，故佐以防己；阴淫寒疾，故佐以附子；阳淫热疾，故佐以黄芩。盖病不单来，杂揉而至，故其用药，亦兼该也。
（3）《成方便读》：方中用麻黄、桂枝、防风、防己入太阳之经祛风逐湿者，以开其表；邪壅于外，则里气不宣，里既不宣，则郁而为热，故以杏仁利之，黄芩清之；而邪之所凑，其气必虚，故以人参、甘草，益气而调中；白芍、川芎，护营而和血；用附子者，既可助补药之力，又能济麻黄以行表也；姜、枣为引者，亦假之以和营卫耳。悲寒无汗者，倍麻黄、防风；恶风有汗者，倍桂枝、白芍；身热有汗不恶寒者，加石膏、知母，减附子；身热有汗不恶风者，加葛根，减附子；身凉无汗者，倍附子；身凉有汗者，倍桂枝、附子。
