Author Description

第六章颈肩腰腿痛的病因学与流行病学
第一节病因学
五、精神因素
此可见于神经衰弱或癔症时。患者的神经活动抑制过程减弱，对疼痛的感受阈降低，肌肉紧张，甚为敏感。颈腰背痛的疼痛性质、部位和范围都不一定，常有变化，压痛位置也常无定点，使人难以捉摸；且体位改变及咳嗽等使腹内压增加的动作，也不会使疼痛加剧。此外尚伴有其他一系列症状，如情绪不稳定、易激动、常感恐惧、容易悲观、不易自制，经常头痛、四肢发麻，记忆力减退等；有时还可伴有呼吸道或胃肠系功能紊乱症状出现，而在腰背部组织或内脏并无任何器质性病变存在。
有时患者会详细叙述腰痛的症状，但查无实据且于腰部结构和功能显然无关，查不出与主诉有关的合乎情理的体征。常见的原因有：
（1）抑郁症和焦虑：患者多遭受重大不幸，如工作的严重挫折，亲人伤病死亡，婚姻破裂等。产生激发性和反应性抑郁症，有特殊的激动、不安、焦虑、紧张、注意力不集中，使脊柱的正常功能、灵活性、应变能力受到干扰、混乱、不协调。因不同肌群运动神经元的活动过强，肌电图却无异常，部分腰背肌无规律的过度运动，导致腰背痛。患者的精神病态会影响主诉的可靠性。患者多先有头痛，在不知不觉中感觉腰痛，或突发腰痛，腰部活动轻至中度受限。
（2）癔症：患者以腰痛为主要症状者，经心理学检验提示，某些持久性腰痛患者的“癔症等级”较高。有的腰背痛时，情绪反应强烈，并明显提示腰背痛与所顾虑的难题，如人事关系、经济利益等相关联系的特点。
（3）诈病：患者常夸张其腰腿痛与某个问题或交通意外相关联，鉴于疼痛是一种主观的情绪有关的紊乱，如病史采集与合乎逻辑的体查，能察觉到诈病的可能性，再进一步做x线检查，与精神病医生的会诊，谨慎作出诈病的诊断。有的诈病也是精神病的一种表现，也需做心理学的分析和治疗。不排除器质性病变，也不轻易确诊为精神病源性腰痛。

本书为“颈肩腰腿痛诊疗系列”第三部，其他两部为《颈肩腰腿痛应用解剖学》和《颈肩腰腿痛应用检查学》。
社会经济的日趋发展产生的社会压力与不良的生活习惯，使颈肩腰腿痛成为一种多发病、常见病。据流行病因学研究结果表明：目前在全世界范围内颈肩腰腿痛已成为发病率最高的职业性疾病。有75％～85％的人在其一生中会发生颈肩腰腿痛。在一些发达国家，如美国腰背痛发生率为15％一20％，欧洲国家腰背痛的发生率为25％一45％。在我国，工人颈肩腰腿痛发病率为70％，办公人员的颈肩腰腿痛发病率为70％～80％。近几年在临床中发现老年退行性变和青少年发病率日趋上升。通过种种数据表明，在当今社会颈肩腰腿痛病已成为严重影响人们的身体健康、正常工作和日常生活的主要原因之一。
所以对颈肩腰腿痛病的诊断与治疗已经迫在眉睫，但目前国内有关颈肩腰腿痛病的专业书籍很少，综合论述颈肩腰腿痛疾病的专业书籍更少，因此编写一部颈肩腰腿痛专业的诊疗学已势在必行。作为本书主编之一的邵福元主任医师以扎实的现代医学理论知识为基础，同时研究了大量的传统医学古典著作，勤求古训、博采众长，在继承中医传统按摩手法的基础上，历经多年临床实践，创立了“邵福元诊断法”和“邵福元无痛治疗法”。本书另一主编邵华磊院长，是“邵福元无痛治疗法”的第一代继承人，从小随父学医，打下了坚实的基础。她广泛学习中外医学的经典著作，认真地比较、分析、吸收每一观点的精髓，结合临床实践经验，继承和发展，使“邵福元诊断法”和“邵福元无痛治疗法”不断完善和提高，目前我院临床治疗中治愈率达81％，有效率达98％。为了使更多的颈肩腰腿痛患者脱离病痛的折磨，为了满足临床、教学和科研的需要，方便从事颈肩腰腿痛专业医师的学习，以及对颈肩腰腿痛疾病的诊断、治疗、预防提供更有效的帮助，我们编写了这本《颈肩腰腿痛应用诊疗学》。
