Author Description

岳云，毕业于第四军医大学医疗系和中国协和医科大学研究生院。曾赴加拿大西安大略大学麻醉科作访问学者。现任首都医科大学附属北京朝阳医院麻醉科主任、教授、博士生导师。中华医学会麻醉学分会常务委员兼秘书长，北京医学会麻醉学专业委员会主任委员，国内外多本杂志的编委、常务编委、副主编。全国卫生专业技术资格考试委员会成员，全国继续医学教育委员会学科组成员。2001年被国务院授予“全国模范军队转业干部”称号，受到中央领导的接见。享受政府特殊津贴。主要研究方向：麻醉与脑功能，器官移植麻醉与危重症监测。承担国家自然科学基金多项。发表论文两百余篇，主编、参编专著二十余部，获多项北京市和中华医学科技进步奖。
吴安石，男，44岁，主任医师，副教授，硕士研究生导师。现担任旨都医科大学附属北京朝阳医院麻醉科行政副主任。1989毕业于湖北医学院医疗系。1994.9-1997.9于中国医科科学院、中国协和医科大学附属阜外心血管病医院获心血管与体外循环麻醉学硕士学位；1999.9-2002.7于首都医科大学研究生院获麻醉学博士学位；2001赴加拿大西安大略大学医学院作访问学者半年；2004赴美国华盛顿大学医学中心作访问学者半年。

Book Abstract

心脏移植受体的病理生理改变主要是心脏的收缩功能或舒张功能障碍。收缩功能障碍主要表现为心肌的每搏输出量降低及心室的舒张末容积增加；舒张功能障碍主要表现为心室的舒张末压力升高。终末期心力衰竭患者机体的交感神经系统的活性增强、儿茶酚胺分泌增加，导致机体全身血管收缩；同时，机体的肾素一血管紧张素一醛固酮系统激活，导致机体的水、钠潴留；随着时间的推移，高水平的儿茶酚胺造成受体密度的降低，导致机体器官（主要是心脏）、血管对此类药物的敏感性下降、心肌细胞的肾上腺素能储备减少。
三、移植前受体的血流动力学评估
移植前受体的血流动力学评估是心脏移植前受体评估的基石。尽管心脏移植受体血流动力学改变最主要表现为左心系统收缩或舒张功能的严重障碍，但拟接受心脏移植患者右心系统的评估，特别是肺动脉压力和（或）肺血管阻力的评估至关重要。终末期心力衰竭患者常伴有不同程度的肺动脉压力升高、肺血管阻力增加，对于肺血管阻力严重增加（≥6Wood单位）拟行心脏移植的患者术前应用肺血管扩张剂行肺血管反应性检查、判断其可逆性与否具有重要的临床意义。经过具有肺血管扩张作用药物的调节，让肺血管达到最大舒张状态，使原本认为不可逆性的肺动脉高压变为可逆、对药物有反应的肺高压，为此类患者的心脏移植带来可能。
