第一章手术室质量安全管理细则
第一节手术室规章制度
一、一般工作制度
1.进出手术室的工作人员，必须严格遵守洁净手术部的封闭管理制度，按照规定线路出入。
2.参加手术的人员必须持胸卡领取手术衣、裤等相关物品，着装符合要求。手术结束后交回领取的物品，取回胸卡。无胸卡者未经允许不得进入手术室。
3.参加手术人员须提前20～30分钟到达手术间，做好术前准备，保证手术准时开始。
4.严格遵守手术室无菌技术操作规程，做到各项操作规范。
5.参加手术者必须在手术申请单上注明。示范教学与手术室护士长联系，根据情况开放电教室。如需进入手术间参观，必须有院领导批示方可入内。
6.为防止发生院内感染，手术间内人数和人员流动必须保持在最低限度，不得随意外出或在各手术间交叉走动，特殊情况必须外出时，要更换外出衣服和鞋子。
7.手术室内应保持肃静，不得大声喧哗，严禁讲与手术无关的话。所有用物定量、定位放置，专人保管，用后归回原处。未经护士长同意，手术室内物品一律不得外借，禁止私用。
8.手术申请单应在手术前日上午10点前通过手术电子预约系统提交手术室，急诊手术由值班医师通过急诊通道发送到手术室的同时，须电话通知手术室护士长或值班护士。手术按手术通知单预约的时间进行，不得随意更改，特殊情况与手术室护士长联系。
9.患者进入手术室必须更换手术衣、帽，由工作人员接入手术室，并带术中用物，如病历、X线片等，首饰、义齿、手表等不得带入手术室。手术结束，由巡回护士、麻醉师、手术医师等送患者回病房，带回用物与病房护士交接。
10.非手术人员不得进入手术室或在手术室洗澡，手术室内禁止吸烟。
11.手术室护士应有高度的责任心，坚守岗位，履行职责，人人参与管理，保证洁净手术部工作的正常运行。
二、查对制度
1.接患者时，手术室工人应与病房护士查对科别、床号、患者姓名、性别、诊断、手术名称、术前用药，分别在手术查对记录单上签字。
2.患者接到手术间后，手术医师、麻醉师、巡回护士分别在麻醉前、皮肤切开前及手术结束患者离开前，按要求进行安全核查，无误后方可进行手术或将患者送出手术室。
3.体腔、深部组织的手术，术前、缝合体腔和缝合切口前、后应清点敷料、器械、纱布垫和缝针等。浅表手术要清点缝针、棉球和纱布等。
4.查对无菌包外3M胶带、包内灭菌指示卡、灭菌有效期及手术器械是否齐全。一次性无菌物品要查对名称、规格、有效期及包装是否完整等。
5.术中取下的标本，洗手护士应与术者核对标本袋上所贴病理条形码、患者腕带与病历上患者姓名、住院号、标本名称等是否相符，无误后洗手护士与术者分别在病理标本登记表上登记、签名后将标本固定好放入指定位置。
