2.误区二 简单地将循证护理等同于将文献综述的结果应用于临床实践。
目前，我国一些公开发表的循证护理临床实践论文存在着严重的对循证护理误解、误用的现象，这些论文报道了就某一特定专题进行文献查询，并将查询到的研究结果直接应用到该专题的临床实践中，误以为这一过程就是“循证”。这些论文存在的问题是：要么并未对查询到的文献质量进行严格评鉴或仅仅简单地一笔带过；要么形式化、简单化地进行文献质量评价而不报道采用的评价标准、评价过程、文献筛选后的结果，却将这一过程冠名为“×××领域的循证护理实践”，这是对循证护理的误解和误用，这种实践方式套用了“循证护理”的名义，但并未正确理解其实质，如果缺乏对文献进行严格的质量评价和筛选，则可能将一些质量低劣，甚至结果不成立的研究结论作为证据应用到临床，从而误导读者、误导护理实践。因此，我们呼吁应该尽快扭转这一学术误区。
由于目前文献的质量参差不齐，因此循证实践过程中至关重要的环节是对文献中查询到的相关研究、专家意见、论断进行质量评价、筛选，该过程称为文献的严格评鉴（Criti Calappraisal）。一旦缺乏对所查询到的各类文献进行严格评鉴，则可能将一些设计存在严重问题、结果错误的研究作为证据应用到临床，这将会导致影响照护结局的不良后果。
因此，如果在寻找证据过程中找到了来自研究、专家经验的结果，应明确文献的类型，如果是研究论文，应明确研究的设计，然后根据循证实践中心的文献质量评价标准（如CO Chrane中心的文献质量评价标准、丁BI循证护理中心的文献质量评价标准）对研究设计严谨性、研究结果的有效性进行质量评价，如果是专家意见和案例报道，应对资料来源的可靠性、合理性、逻辑性、临床意义进行严格评鉴。上述过程应详细报道（如文献质量评价采用的标准、评价的过程、筛选的结果），才能判断该结论的可利用性。
文献质量评价是循证实践的关键，但是该过程比较繁琐费时，对于临床一线的护理人员而言往往没有时间进行上述过程，因此目前循证实践领域推荐的做法是直接查询已经进行了严格评价的、成熟的循证资源，如系统评价类文献、临床实践指南，或者最佳实践报道，这些资源都是经过循证实践机构的专业人员进行严格评鉴后形成的循证资源，可供临床护理人员应用，但是也应注意结合自身的临床情景和患儿的需求选择适合的证据。
目前，在循证护理领域的系统评价、临床实践指南、最佳实践报道等资源在国外比较丰富，但中文版的这类资源尚不多，有待将国外的循证资源翻译、引进我国，或者形成我国本土化的系统评价、临床实践指南、最佳实践报道。
