姚礼庆，男，1951年出生。复旦大学附属中山医院内镜中心主任、教授、博士生导师、主任医师；复旦大学内镜诊疗研究所所长；复旦大学大肠癌研究中心顾问；卫生部内镜诊疗技术（上海）消化科培训基地和考试中心主任，兼任卫生部内镜专业技术考评委员会委员；中华医学会消化内镜学分会委员、外科学组组长；中华医学会结直肠肛门外科学组委员；上海市消化内镜专业委员会主任委员。擅长胃食管静脉曲张的内镜治疗、消化道狭窄的内镜下扩张和内支架治疗、经内镜逆行胆胰管造影术、消化道肿瘤的内镜治疗、胃肠道癌症的外科治疗和吻合器痔上黏膜环切术。担任《中华胃肠外科杂志》、《中华消化内镜杂志》、《中国实用外科杂志》等十本杂志编委。发表论文一百八十余篇，其中被科学引文索引（SCI）收录十篇，科普文章七十余篇；主编《现代内镜学》、《外科手术并发症的预防和处理》、《内镜黏膜下剥离术》等专著八部，参与编写著作二十本。2008年获得复旦大学校长奖，2009年获恩德思医学科学技术奖——中国内镜杰出领袖奖，2010年获得上海市医学科技奖一等奖。
陈巍峰，男，1979年出生。外科学博士，复旦大学附属中山医院普外科、内镜中心主治医师。主要从事消化内镜和普外科工作，擅长各种消化道疾病的内镜诊断与治疗，包括消化道息肉、早期癌和黏膜下肿瘤的内镜下诊断与治疗，消化道良恶性狭窄的扩张及支架治疗，胆胰疾病的内镜逆行胰胆管造影术治疗等。其中“内镜黏膜下剥离术和黏膜下挖除术治疗消化道早期癌和黏膜下肿瘤”获得上海市医学科技奖一等奖（第五完成人）。担任《胃肠病和肝病学杂志》特约编委，近年先后在国内外核心期刊上发表医学论文二十余篇，参加《现代内镜学》、《下消化道内镜学》、《外科手术并发症的预防和处理》、《内镜黏膜下剥离术》等医学专著的编写。

Book Abstract

第三节胆胰管引流器械
对于结石、炎性狭窄、胆管癌等所致的胆管内阻塞，以及肝癌、胰腺癌及转移癌等恶性肿瘤所致的胆管外压迫所致胆道梗阻，胆道支架植入到狭窄或阻塞部位或是留置鼻胆管是目前较好的方法。
胆道支架按其材质和用途，分为临时陛的塑料支架和永久性金属支架两种。
一、胆道塑料支架
胆道塑料支架用于治疗胆胰管良性狭窄、癌性狭窄的术前减轻黄疸，急性化脓性胆管炎的胆道引流等。塑料支架需与支架推送器（包括一个推送套管和一个导引导管组成）配合使用。目前许多公司推出一体式胆道支架系统，由一根共轴的聚全氟乙丙烯材质的导引导管和一根坚硬、可以推送的定位器组成，两者均与一个手柄连接。一体式支架的优点在于输送系统内预装支架使医生能够重新定位支架，帮助确保支架放置于准确的位置（图2-3-1、2）。
二、胆道金属支架
金属支架主要用于原发性或转移性晚期恶性肿瘤引起的恶性胆道梗阻，引流胆汁，减轻黄疸，改善患者生活质量。金属支架完全扩张后的口径可达7～12mm，远非传统塑料支架可比，加之金属材料光洁度较高，并可为胆道上皮细胞覆盖，使得细菌和胆泥物质不易黏附，因而不易发生阻塞和移位，持久通畅性能较佳。
目前应用最广泛的胆道金属支架是钛镍形状记忆合金丝编织成网状的自膨式支架，放人不大于10F的推送器内，操作医师将推送器经内镜管道放至胆胰管狭窄部位后，在x线的监视下，退出推送管，释放支架，在释放过程中根据x线监视图像，不断调整支架位置，使支架位置完全覆盖狭窄段，起到支撑作用。
