Book Abstract

肾小球滤过屏障对水有着高度通透性，其中基底膜和裂孔隔膜对水有一定的阻力。在实验性肾病综合征中发现足突融合使得肾小球滤过屏障对水的通透性明显降低。而肾小球滤过屏障通过分子大小、电荷、分子形状三方面来选择性限制大分子物质的通过。电荷选择性是基于基底膜、内皮细胞及足细胞表面覆盖的大量带负电荷的蛋白多糖。分子大小选择性部分取决于基底膜的致密胶原网络，但主要在于裂孔隔膜部位。有效半径4nm的分子完全受阻（白蛋白的有效半径为3.6 nm）。基底膜及裂孔隔膜对肾小球选择通透性的重要性早已被证明，针对两者结构蛋白（如蛋白多糖、Ⅳ型胶原、nephrin等）的特异性抗体可以诱发严重蛋白尿。早期一些不同电荷及大小的分子筛选系数的研究表明基底膜同时作为分子大小及电荷的选择性屏障。但随着研究进展，裂孔隔膜被认为是更重要的屏障。基底膜上Ⅳ型胶原蛋白a3～as链的基因突变（导致Al-port综合征和薄基底膜肾病）及硫酸肝素链的基因突变并不引起肾病范围的蛋白尿。但近来这种以足细胞为中心的观点又受到挑战，研究发现基底膜上层黏蛋白B基因的突变可引起大量蛋白尿，尽管裂孔隔膜结构完好。这种研究结果上的反复也加深了我们的认识，可以推断肾小球滤过屏障的3个部分是相互联系、共同发挥作用的，很难说哪层结构更重要。
