Book Abstract

因为大辛温热之药固然可以温阳驱寒，辛香苦燥之药固然可以流通气机，但径情直往，或者反复使用，每能耗散气阴，产生流弊，不但起不到治病的应有作用，相反正气愈伤，病情变得愈为复杂，所谓“寒病未已，热病复起”，处理更为棘手。如其配伍适当的甘柔顾阴药，预为防范，就不但可以纠正弊病，而且相得益彰，有助于阳气的恢复和气机的流通。至于配伍的阴柔药，当然尽可能选择恰合病情的品种，可以起到协同作用，加强疗效。但有时亦不完全如此，仅仅取其监制或调剂而已。这两种刚柔相济的用药，都是相辅相成的。
具体配伍，作为调补阴阳者，严用和早有阐发，如在《济生方》卷一补益门中说：“凡人有虚损之病，及早为之补益，庶有延龄之望。但须药性平补，柔而不僭，专而不杂。间有药用群队，必使刚柔相济，佐使合宜，可以取效。前贤之书，有单服附子之戒者，正虑其肾恶燥也。既欲用一刚剂专而易效，须当用一柔剂以制其刚，则庶几刚柔相济，不特取效之速，亦可使无后患。……用药在乎稳重故也。”例如附子、肉桂配伍熟地、山茱萸肉等，用于八味丸，即名之为肾气丸，而不日补阳丸。又如用破故纸、杜仲配伍胡桃肉、熟蜜，名之为青娥丸，而不日壮阳丸，均具有深意。至于张景岳右归饮、右归丸的配伍方法，就更明显可知了。
又如巴戟与苁蓉，沙苑蒺藜与枸杞子，续断与枸杞子，沙苑蒺藜、菟丝子与五味子，菟丝子与熟地，杜仲与牛膝，巴戟肉与牛膝（巴戟天酒），鹿角与牛膝（鹿角丸）等配伍，均属同样意义，尽管用药有所不同，但都能温润补肾，叶天士称之为“温柔”方法，常用于下焦阳虚，而又兼阴精不足之证者。
尚有破故纸配伍胡桃肉，破故纸配伍熟地，桂心配伍熟地，沉香配伍熟地等，又可以温纳肾气，治疗肾虚喘逆等证。
