Book Abstract

《养肝就是养命》：
正常的人体肝脏呈红褐色、楔形，位于人体右季肋区以及腹部上区的位置，被腹膜褶皱形成的肝周韧带固定在上腹部的位置，包括肝圆韧带、镰状韧带、冠状韧带、左右三角韧带等。大部分肝为肋弓所覆盖，仅在腹上区、右肋弓间露出并直接接触腹前壁，肝上面则与膈还有腹前壁相接。一般来说，成人的肝脏上界位置正常的情况下，如果在肋弓下触及肝脏，那么极有可能就是病理性肝大。幼儿时期的肝下缘位置一般都比较低，露出到右肋下一般均属于正常情况。
肝脏的位置时常会随着呼吸而改变。通常，平静呼吸时，肝脏位置的升降可达2～3厘米；站立吸气时，稍微会有所下降；仰卧呼气时，则稍有上升。医生在给患者进行肝脏触诊检查时，通常会要求患者做呼吸配合，就是这个道理。
肝脏由肝细胞组成，人体肝脏约有25亿个肝细胞。肝细胞呈现出多角形的状态，有6～8个面，直径为20～30微米，体积极其微小，约4900平方微米，必须通过显微镜才可以观察到。在不同的生理条件下，肝细胞的大小存在着一定的差异，例如，饥饿时肝细胞的体积会变大。每个人体肝细胞表面分为窦状隙面、肝细胞面和胆小管面三种。肝细胞里面含有许多复杂的细微结构，如肝细胞核、肝细胞质、线粒体、内质网、溶酶体、高尔基体、微粒体以及胞饮液泡等。每一种细微结构都有着极其重要并且非常复杂的功能，正是这些功能保证了我们生命的存在，保证了我们能够活下去。
以肝内血管和肝内裂隙为基础，肝脏可以分为五叶、四段，即左内叶、左外叶、右前叶、右后叶、尾叶；左外叶又分为左外叶上段和左外叶下段，右后叶又分为右后叶上段和右后叶下段。而每5000个肝细胞又组成一个肝小叶，人体内的肝小叶总数约有50万个。
肝脏对维持生命活动的正常进行有着极其重要的作用。首先是代谢功能，包括糖、脂肪、蛋白质等营养代谢以及多种维生素、矿物质等的代谢，源源不断地为机体供应能量；其次，肝脏具有解毒功能，可以保护机体不受毒害；另外，肝脏具有制造消化系统中胆汁的功能，支持脂肪的消化。此外，肝脏的功能还有：肝脏中的免疫细胞生成免疫成分，排出血液中的细菌，分解衰老的红细胞以及废弃的激素；储藏红细胞所需的铁分，用于制造血液；储藏维生素类或进行活用……据说，肝脏具有200多种功能。由此可见，肝脏算得上是人体当中最为强大的脏器，是一位名副其实、功不可没的“大将军”。
需要注意的是，肝脏这个威武的大将军一直担负重任努力战斗，因此也非常容易“负伤”，很容易患上肝炎、肝癌或肝硬化等疾病。其中，最为严重的便是肝癌。肝癌的早期表现并不典型，但是却发作快，死亡也快。一旦患上肝癌，患者通常都是九死一生。
患者可能会有食欲下降、腹胀、胸闷、感冒、发热、呕吐等症状，但是平常人都不会把这些症状当回事，常误认为是饮食不当、着凉等原因所造成的，然后就自己到药店买药吃，导致了病情的恶化，直到最后发现却为时已晚。肝病的可怕之处在于包括肝癌在内的大部分肝病都没有特别显著的证状，这也就是医生称肝病为。“最大的隐形杀手”的原因所在。
正是由于肝脏具有如此众多并且重要的功能，又起病隐匿、病情危急，所以，古今名医都相当重视对肝脏的养护，以呵护这位功不可没又容易“负伤’’的大将军。
肝脏由肝以及一系列管道结构组成，血液供应非常丰富，其血容量相当于人体总量的14％，成人的肝脏每分钟的血流量为1500-2000毫升。
