Book Abstract

第3天舌诊悠久的历史
《黄帝内经》（包括《素问》和《灵枢》两部分）是目前我国最早的系统的中医典籍之一，是一部综合论述中医理论的经典著作。其中对于舌诊的解剖、原理、经脉所主多有涉及，但其对舌诊论述、应用远没有后世系统。
汉代名医张仲景被后世尊为“医圣”，其所著《伤寒杂病论》（后被分为《伤寒论》《金匮要略》两书）为后世医家所效法，其中提到舌诊条文30余条，下面摘取几段典型条文，窥其一斑。
《金匮要略．腹满寒疝宿食病脉症并治》：“病者腹满，按之不痛为虚，痛者为实，可下之，舌黄未下者，下之黄自去。”
这里提到腹胀，如果用手按，不痛多为虚证，按压痛多为实证，对于实证，可以用攻下驱邪的方法，像这样的人一般舌苔多为黄色，如黄而老、黄而燥，用下法将邪气驱除，黄苔也就慢慢消退。
《伤寒论》130条：“脏结无阳证，不往来寒热，其人反静，舌上苔滑者，不可攻也。”
“脏结”是《伤寒论》里的一个病名，可以简单地理解为人身之气结聚在一起，症状表现为胸骨下、胃脘区按压痛。既然是结聚就要用中医“辛开苦降”的药物把这股结聚之气打开，这是常规思路，但病人此时表现出“不往来寒热”，有另有一个版本说是“但寒不热”，总之是看不到人体自救的机制，“其人反静”，病人少气懒言，毫无生机，此时的舌象为“舌上苔滑”，舌面上看上去澄清并且水液欲滴的样子，这样的舌象是主阴寒、主阳虚、主水饮的，正如前面所说“无阳证”。此时矛盾出来了，脏结需要开破，但气虚又不可开破，因此仲景说不可攻也。对于这段话，推而广之可以为肿瘤的治疗提供一点思路，一般来说，肿瘤需要活血化瘀、开瘀散结，但有些阳虚病人并不适合。当然，即便是有些可以的，中医还有另一句话，“大积大聚，其可攻也，衰其大半而止，过者死。”充分体现了中医是治“病的人”，而非治“人的病”，不会为了“大积大聚”而去害人的正气。

本书作为一部中医舌诊科普书籍，从中医理论、发展历程、实际应用等方面，通俗易懂地介绍了生活中常用的中医识别舌象的方法。本书既可供中医院校学生在舌诊教材之外深入学习，也可供临床医生诊断疾病参考，还可为广大中医爱好者提供相关知识，指导日常生活保健。
本书从内容上说，分为舌诊总论与舌诊各论两大部分。舌诊总论介绍了舌诊在中医诊断中的地位、舌诊的发展历史、舌诊与脏腑经络的关系、舌诊的方法与注意事项、舌诊的现代研究等，主要对应本书第一周的内容。舌诊分论将舌诊从舌色、舌形、舌态、苔色、苔质、舌诊与饮食口味、舌诊与病证、全面分析舌象等几个方面分别论述，主要对应本书第二至十周的内容。在“病证与舌象”内容中，我们选择了感冒、哮病、汗证、不寐等14个病证，分2周讲述，在具体的病证中学习舌诊，可以更加深刻理解中医辨证论治的特色以及舌诊在诊断中的作用。在”辨析口味”一节中，我们特别将问诊中“问饮食口味”的内容加入其中，增加了舌诊的完整性与实用性，也算是本书的一大特色。最后我们将临床过程中拍摄到舌诊的图片精选出12张附在本书的末尾，供读者在学习舌诊的过程中参考，虽然仅仅是12张，但已经大致包括了临床过程中常见的基本舌象。
中医学博大精深，科学技术日新月异。由于编者水平有限，时间紧迫，书中难免出现缺点和错误，敬请读者批评指正，以便再版时修改完善。
