Book Abstract

无论是古今文献的大量记载、中医临床经验的积累，还是运用现代科学实验方法，都证实了循经感传及其他经络现象的客观存在。近几十年来，已有多位学者采用多学科的方法对经络实质进行了全方位的探讨和研究。目前对于经络的结构，较为成熟的假说有以下几种。
经络已知结构论
根据《黄帝内经》等古医籍中关于古代医家对尸体的解剖观察和对经脉、脏腑形态的记载，以及对经脉循行、功能等的论述和循经感传的大量研究，许多学者认为，经络是客观存在并有其形态基础的，它可能存在于已知的组织结构中；它是以神经、体液系统，包括血淋巴系统和人体间隙系统等在内的人体功能调节系统为主要结构基础。
与血管有关：血管、微血管网络，是经络的组成部分，约45.5％的穴位位于大血管周围，其中18.6％的穴位位于血管上。经脉所通过的部位以及穴位部位均与血管分布状况有一定的相似性；通过血管壁的交感传人和交感反射功能，实现接受和传布信息的作用。
与神经相关：与中枢或周围神经系统相关，十二经脉与外周神经有非常密切的关系，经线上的各种神经成分（神经束、神经纤维、神经末梢）等均较非经线的对照部位多。
与淋巴相关：十二经脉与淋巴相关，督脉、任脉和带脉与淋巴结收集丛有关。经络多同淋巴管走行一致，在四肢、躯干及胸腹部，微细淋巴管集柬的地方多有穴位。针刺或艾灸对淋巴液的流速和淋巴功能的改变有相当大的影响。
与肌肉相关：观察表明人体一些骨骼肌首尾相接，其分布位置与经脉的循行路线非常相似，并在一部分感传显著的志愿者身上记录到了与感传同步的肌电反应。54.8％的穴位处于肌肉群上。
综合经络结构研究的主要观点，不外乎以下几种：一是有学者认为，经络不是由一种组织结构组成，而是由多种组织共同构成的一个多层次的空间结构，“人体间隙系统说”就是这方面的代表。二是有研究表明，对针灸机制和“经络”概念，可用神经体液综合调节机制来阐述，其观点源于对针刺效应和穴位相对特异性等问题的研究。三是有文献指出，经络联系可能是以体表内脏植物性联系为主，并认为古人所说的经络就是指人体的神经和循环两大系统，前者为联系系统，后者为运输系统。总而言之，上述观点均强调经络是已知的结构和功能。

Introduction

中国的医学，自古以来便以其独特的思维方式来解释穴道的治疗效果，在《黄帝内经·素问》中就有记载“气血不顺百病生”的句子。所谓的“气血”，就是指支配内脏的一种能量，而这种能量若流动混乱，就会引起各种疾病。穴位就位于能量流动的通路上。

善用经穴的按摩，不仅可刺激经穴、改善气血，还可强化肌肉、关节，增加血液及淋巴循环系统流畅，同时促进机体的生理效应，改善病理过程，进而减缓疼痛，调节免疫力，提高自然抗病力，以至能达到预防疾病或促进康复的目的。

中医认为人体是以五脏为中心，通过经络系统，将六腑、五体、五官、九窍、四肢百骸等全身组织器官组成一个有机的整体，并通过精、气、血、津液的作用来完成机体统一的功能活动。头为元神所居、诸阳之会；脑为精明之府，又为髓海所在之处。因此，头脸部若发生疼痛等病症，则是“牵一发而动全身”。

肩颈病痛属于痹症范畴，由外伤劳损、外感风寒湿之邪而引致，使气血运行不畅、气滞血淤，不通则痛，筋肌失养，而不能束约骨骼和稳定关节，以致产生骨错逢、筋出槽，如肩颈肌肉痉挛、酸痛、关节强直等表现。
