Book Abstract

《亚洲人鼻整形术》：
作者于10多年问共施行1000多例膨体鼻整形术，总感染率为1.3％。其中使用膨体为鼻小柱支撑移植物的10例患者多数继发了感染（图3—15），如果去除这部分病例，膨体用于鼻背部隆鼻的感染率仅为0.5％。作者认为，假体的感染率差异不在于假体本身。预防性使用抗生素与否、消毒处理方式及术者的经验和操作技巧等诸多因素均影响感染的发生。
有些人认为，因内部的气孔结构，膨体的远期感染率和亚临床感染率高于其他假体（图3—16）。细菌黏附在气孔内部，远期当患者免疫力低下时，细菌缓慢增殖并诱发感染和亚临床感染。膨体气孔大小为10～30μm，巨噬细胞（30～40μm）很难通过气孑L，但大小为0.5—1.5μm的细菌很容易通过。早期细菌黏附在气孔内部可以逃过免疫系统的监视（图3—17），故使用气孔型假体（膨体）时，一定要严格遵守无菌原则。但即使是无孔型假体（硅胶），远期也可能并发感染（图3—18）。
有气孔的膨体，一方面因不形成包膜、与周围组织粘连等原因，增强了抗感染能力；另一方面，因细菌可以残留在气孔内部并诱发远期感染。如果忽视膨体的这一特点，不严格遵守无菌原则，初次使用膨体的患者不可避免会频繁出现感染并发症。
由此得出结论：术者如能严格遵守无菌原则，膨体和硅胶的感染率无显著差异。
为了矫正重度鞍鼻，鼻背部需要植入较高的假体。无论是硅胶或膨体，这时都将增加感染的可能性，并且出现皮肤变薄、假体轮廓的显现等并发症。为避免这些并发症，重度鞍鼻患者更适合使用肋软骨等自体软骨移植物充填鼻背部。
（二）外形不自然
部分患者术后外形不自然，易让人察觉曾接受过鼻整形术。大多是由于皮肤发红、假体轮廓的显现及皮肤变薄等症状所致。一般认为膨体术后外观自然，质地柔软的硅胶外观也不易被察觉，但这种见解不完全正确。
任何假体术后都可能出现外形上的不自然，没有任何假体是对所有患者都是适合的（图3—19）。图3—20是硅胶隆鼻术后出现皮肤变色的患者，二次手术时使用膨体取代缓解了症状。
