解表方剂知多少
中医学认为，肌表是人体抗御外邪的第一道屏障，口鼻内通于肺，肺外合皮毛而主表。所以外邪侵袭人体，首犯肌表肺卫，出现恶寒发热、头身疼痛、鼻塞咳嗽、脉浮等表证。此时因病邪轻浅，应遵照《黄帝内经》“因其轻而扬之”，“其在皮者，汗而发之”的治疗原则，使邪气从肌表而出。如果不能及时治疗，或治疗不得法，外来邪气不能及时从外解散，势必要转而深入，变生其他的病证。因此，外感初期，及时地使用解表剂治疗，不仅使邪从外解，还能防止病邪传变入里，使疾病早期获愈。
解表剂以解表药为主组成，具有发汗解肌，疏达腠理，透邪外出等作用，能够治疗表证，就是通过开泄腠理、调畅营卫、宣发肺气等效应，使在表的外感邪气随汗出而解。
解表剂主要用于解除表证，但表证不能单纯地理解为通常所说的感冒，还包括麻疹、疮疡、痹证、水肿、疟疾、痢疾等初起兼有表证者。如麻疹主因时疫邪毒外发肌表所致，若肌表受邪，邪毒外发不畅，可致疹出不畅，甚至有内陷的可能，解表剂可发散外邪、疏畅肌表，可助疹毒由肌表向外透达；水肿初期兼有表证者，多与外邪犯表，肺失肃降，水道不利有关，解表剂能发散外邪而开玄府（即汗孔毛窍），宣通肺气而利水道，有助水液从汗液、尿液外排；肌表疮疡，初起多有发热恶寒等表证，其病机常涉及邪毒犯表，营卫壅滞，解表剂能发散外邪，宣通营卫，有助疮疡的消散；痹证是风寒湿邪侵袭人体导致的，风湿在表可出现头身肢体疼痛、沉重的症状，有“医圣”之称的东汉医家张仲景在《金匮要略》中说：“风湿相搏，一身尽疼痛，法当汗出而解”，并进一步指出，发汗宜“微微似欲汗出者”，才能使“风湿俱去”。解表剂用于痹证初起，具有祛风散湿的功效；痢疾因外邪不解，内陷肠腑，或疟疾因外感时邪引发者，解表剂既可解除表邪，还可升散透发，可助里邪外达出表。
一、解表剂的分类
外感六淫之邪有寒热之异，人体有虚实之别，临床表证主要有表寒和表热以及虚人外感几种类型，分别治宜辛温解表、辛凉解表及扶正解表。所以，解表剂一般可分为辛温解表剂、辛凉解表剂和扶正解表剂j类。
1.辛温解表剂
适用于由外感风寒引起的发热、恶风寒，头项强痛、肢体酸痛、口不渴、舌苔薄白，脉浮等表证，常用辛温解表药如麻黄、桂枝、荆芥、防风、紫苏叶、羌活等为主组成。风寒表证中，或邪犯肌表，肺失宣降；或风寒夹湿，经络阻滞，或肌表被郁，肺胃气滞；或阳盛之体，邪从热化；或素有寒饮，内外合邪而有各种伴随兼证。故本类方剂又常配伍宣肺止咳、除湿通络、理气行滞、清里热、温化痰饮等药味。代表方如麻黄汤、桂枝汤、九味羌活汤、小青龙汤、止嗽散、香苏散、杏苏散。
2.辛凉解表剂
适用于外感风热表证，简称表热证。表热证的主要临床表现为发热，微恶风寒，头痛咳嗽，口渴咽痛，舌苔薄白或微黄，脉浮数等。临证常用辛凉解表药为主组方，如薄荷、银花、桑叶、菊花、牛蒡子、葛根等。外感表热证中，邪从口鼻侵入，直犯上焦，或咽喉不利，或肺失宣降；或上焦蕴热，热伤津液；间有疹毒外发，邪郁肌表，疹发不畅等证，所以本类方剂又常配伍解毒利咽、宣肺止咳、清热生津、解毒透疹等药味。代表方如桑菊饮、银翘散、麻杏甘石汤等。
3.扶正解表剂
适用于体质素虚又感外邪所致的表证。此时既要解表，又要考虑到正虚，因为此时汗源不充，如果只用解表药，正气无力辅助药力驱邪外出，所以有时单服解表药却无效。因此，虚人有表证，应当扶正解表，常用益气、助阳、滋阴、养血药与解表药配伍组成方剂。此类方剂配伍扶正药并非单纯为治虚而用补药，更不是使正虚得以恢复，而是为了扶助正气，以增强祛邪的能力，所以补益药的剂量应根据正虚的轻重程度来定，不能认为扶正之力越大就越好，特别是素体多痰湿者，尤其要注意，以防止扶正不成，反助病邪。这时应该做到解表不伤正，补益不恋邪。由于外邪有寒热之别，虚人常有兼夹病邪，如阳虚生内寒，气虚多痰湿，阴虚生内热，血虚多津少，应当细审病机，组方配伍中可酌情兼顾。代表方如败毒散等。
